Use of School Facilities
All requests for use of a Pembroke Public Schools facility must be completed online. The online form reserves your space on our calendar, pending event approval. You must complete all steps below in order for your request to be considered for approval.

- Review the fee structure by clicking here.
- Submit your online registration form by clicking here. The online submission password is "titans". Instructions for submitting your online request are available here.
- Submit a printed and signed "Release of All Claims and Liabilities" form by downloading the PDF below.
At the time of your event, all participants must also sign the "Release of All Claims and Liabilities" form. All signed forms must be returned to Katie Eakins at the address below.
